NEO Troubled Families resolves the challenge of sharing information across a multi-agency partnership. Now social services, health, housing, police and voluntary sector organisations can securely share information to underpin ‘key working’ troubled families and monitor the results of the multi-agency team support. NEO Troubled Families enable some partner systems to connect to share data and cut out data duplication.
This application is designed to support the Payment by Results initiative for local authorities to get children back into school, reduce youth crime and anti-social behaviour, put adults on a path back to work and to reduce the high costs these families place on the public sector. NEO Troubled Families includes features which support joining up of local services to manage each problem as a whole and supports a single key worker approach to coordinate services.
Key workers across multiple agencies access a holistic view on the family and have the ability to track interventions, record outcomes or significant changes and monitor reports on outcomes. Progress reports for submission to the DCLG are easily generated, providing a simple but critical solution for managing payment by results.
